Unexplained infertility: when every test is normal
Unexplained infertility is the term used when a full work-up — semen analysis, confirmed ovulation, open tubes, hormone tests — comes back normal for both partners, and pregnancy still has not followed. It does not mean nothing is wrong, only that today's tests cannot show what it is. Heartbeats IVF treats this at all four of its Goa clinics, usually starting with IUI and moving to IVF if needed.
A "normal" report can feel like being disbelieved. You wanted a reason, almost more than you wanted reassurance — something to point at and treat. It helps to know the label describes the tests, not your body.
What "unexplained" actually means
It applies when pregnancy has not happened after trying for genuinely longer than is usual. All the standard tests come back normal: a normal semen analysis, confirmed ovulation, open fallopian tubes, and no other obvious finding on the work-up. Something real is very likely happening — an egg or sperm quality issue, a subtle problem with implantation, or timing that keeps missing by hours despite everything looking right on paper. Current tests simply cannot see it.
Making sure "unexplained" is actually earned
Before accepting the label, we make sure the work-up behind it is genuinely complete — see our page on fertility tests. Sometimes a couple has been told "unexplained" without ever having had an HSG or a full hormone panel, and the label was premature.
Treatment that still works, even without a clear cause
- Ovulation induction with monitoring — even when you already ovulate normally, this can help by improving timing. It can also occasionally ripen more than one egg, so twins become a possibility; that is exactly why we scan through the cycle, and we call a month off if too many follicles are growing
- IUI — a reasonable, less intensive first step for many couples
- IVF — if IUI does not lead to a pregnancy after a fair try, IVF lets the laboratory see the egg, the sperm and fertilisation itself directly, information no work-up alone can give. Sometimes it quietly explains what "unexplained" could not — eggs that do not fertilise well, for instance, which ICSI can then address
What we won't do
We will not use "unexplained" as a reason to push you straight to the most expensive treatment, and we will not pretend a cause exists when it does not. We go in order, honestly, checking at each step whether it is actually necessary for you.

Questions couples ask about unexplained infertility
Can unexplained infertility be treated if we do not know the cause?
Yes. IUI and IVF both work without a known cause, and IVF can even reveal one along the way.
How long should we try naturally before being called unexplained?
Long enough that ordinary bad luck becomes unlikely. If you are already worried, come in and let the tests decide rather than waiting further.
Could stress be the cause?
It is rarely the actual cause of infertility, though it is a completely reasonable thing to feel while going through this. Testing finds what is missed, not guessing.
Should we go straight to IVF since everything else is "unexplained" anyway?
Not necessarily. IUI is a reasonable, gentler first step for many couples, and we will tell you plainly if your situation suggests skipping it.
Is unexplained infertility our fault?
No. It means today's tests cannot show a cause, not that either of you did something wrong.
Will IVF definitely find the reason?
Not always, but it often gives more information than any test could, since it lets us see fertilisation itself happen.
