Secondary infertility: trouble conceiving again

Secondary infertility means difficulty conceiving again after already having a child with the same partner, with no earlier trouble conceiving. It is common, often surprises couples who assumed it would be easy the second time, and usually has a checkable, sometimes treatable, cause. The work-up is the same set of fertility tests used for any couple, done at whichever Heartbeats clinic is nearest you — Panjim, Margao, Mapusa or Vasco.

This problem carries its own quiet isolation. Because you already have a child, people assume conceiving is easy for you and sometimes say so out loud — "you already have one, be grateful" — without meaning harm, and without understanding that the struggle now is just as real.

The good news, and the honest caveat

Having already conceived once genuinely rules out some of the more serious explanations, which is useful information. But it does not prove that everything is still fine now. Testing is still the right first step, rather than assuming this will resolve on its own the way it did before.

What the work-up looks like

The same core fertility tests, for both of you. It is worth saying plainly: it is easy for a couple to assume this is "her problem again," when very often it is not, or not only. A fresh semen analysis is a fair and useful ask, not an accusation.

Treatment

Often simpler than couples expect. Correcting an ovulation problem, or addressing a mild male-factor finding, may be enough on its own. IUI is a reasonable first step where the tubes are open and the semen report is reasonable. IVF is the honest route where a tube is blocked, or where simpler options have not worked after a fair try.

Questions couples ask about secondary infertility

Is secondary infertility as serious as never having conceived?

It deserves the same proper work-up. Having conceived before does not rule out a new cause developing since.

Why is it harder to conceive now when it was not before?

Time and circumstances change. Age, weight, a complicated delivery or infection, or a new finding in either partner can all be reasons.

Should my husband repeat a semen test even though we already have a child?

Yes. Count and motility can decline over the years, so a fresh sample is worth checking rather than assuming the old result still holds.

Do we need IVF for a second child?

Not necessarily. Many couples need a simpler treatment first. IVF is used when the tubes are blocked, or simpler options have not worked.

Is it normal to feel guilty about struggling with this when we already have one child?

It is a common feeling, and an understandable one. The difficulty is real and deserves proper attention regardless of what anyone else says.
